Before your appointment, download Form I-693 from the USCIS website and complete Part 1 only. Bring this form along with your vaccination records, government-issued ID, and any previous medical documentation.
During your first visit, our medical team will begin the physical examination, review your vaccination history, administer any required vaccines, and perform necessary lab tests (TB, syphilis, gonorrhea screening). The entire process typically takes 1 hour.
During your second visit, a USCIS-designated civil surgeon will review all exam results, complete the remaining parts of Form I-693, and sign the sealed envelope.
Receive your sealed I-693 form (do not open it!) and submit it. The sealed envelope ensures the integrity of your medical examination results and maintains USCIS compliance.

The completed I-693 is valid for 2 years from the date the civil surgeon signs it. We recommend completing your exam no more than 60 days before submitting your adjustment of status application to USCIS.
Yes, immigration physicals are conducted under the direction of civil surgeons per USCIS requirements. Immunizations and testing may be done by nurses or other qualified staff, but document review and approval will be conducted by a USCIS-designated civil surgeon. Each exam is performed by a civil surgeon with extensive knowledge of immigration medical requirements, ensuring compliance and approval.
USCIS requires age-appropriate vaccinations. These can include: measles, mumps, and rubella (MMR); polio; tetanus, diphtheria and pertussis (Tdap); Hib; hepatitis A; hepatitis B; rotavirus; meningitis; varicella (chickenpox); pneumonia; and flu. These immunizations are given following ACIP and USCIS guidelines. Some vaccines may not be required based on your age or current medical condition. Bring your vaccination records to your appointment - we'll help determine which immunizations you still need.
If you don't have records, we can either administer the required vaccinations or perform blood tests (titers) to check for immunity. We'll work with you to find the most appropriate and cost-effective solution.

Immigration physicals are typically not covered by insurance as they're considered an administrative requirement. However, we offer competitive cash pricing and accept all major credit cards. Some vaccinations may be covered if you have insurance.
